1. Do you ever find yourself spending an inordinate amount of time thinking about food?

2. Do you ever find that much of your day is spent thinking about or obsessing over your weight and/or body shape?

3. Have you gone to great lengths to rid your body of food eaten? (e.g. abusing laxatives or diuretics, intentionally vomiting/purging)

4. Have you experienced tooth discoloration or decay due to purging behaviors?

5. Do you believe that you may have lost control over how much and how often you eat?

6. Does your weight fluctuate often?

7. Do you experience shame after eating?

8. Do you consider certain foods off limits?

9. Are you intensely fearful of weight gain?

10. Are your glands frequently swollen?

11. Do you have digestive problems?

12. Are you plagued by feelings of anxiety, depression, low self-esteem, or self-worth?
